New Family

I,do hereby consent and agree that Unity North Atlanta Church (UNA), through its employees, members, agents, independent contractors and others authorized by UNA, have the right to take photographs, videotape or digital recordings of myself and/or the minor child or minor children listed below and to use these in print and electronic form as well as in any and all other media, now or hereafter known. I further consent that my and my minor child or my minor children’s names and identities may be revealed therein or by descriptive text or commentary.

I do hereby release to Unity North Atlanta Church and its employees, members, agents, independent contractors and others authorized by Unity North Atlanta Church all rights to exhibit the photographs, videotape or digital recordings in print and electronic form as well as in any and all other media, now or hereafter known, publicly or privately, and to market and sell copies. I waive any rights, claims or interests I may have to control the use of my, my minor child or my minor children’s identities or likenesses in whatever media used.

I understand that there will be no financial or other remuneration for recording me, my minor child, or my minor children, either for initial or subsequent transmission or playback.

I attest that I am the parent or legal guardian of the minor child or minor children listed below and that I have full authority to consent and authorize Unity North Atlanta Church, through its employees, members, agents, independent contractors, and others authorized by UNA to use their likenesses and names. This consent will remain in effect until I request that it be changed or rescinded.
